Question: Soap notes massage therapy?

I am a massage therapy student, on our last deep tissue class we palpated rectus abdominis and obliques, i have to do a soap note creating an issue to a client , to treat these muscles what kind of pain and relation the client have to have...and how do i complete a soap note? Thanks for your help

S -- what is the client complaining of-- O--- what do you feel or palpate on the client A- what do you think is wrong with the client P--- what is your plan to get the client better abdominal muscle complaints are very similar to other muscle complaints so just use this guideline and other muscle symptoms to complete a soap
